GENERAL REGISTRATION INFORMATION
Spring 2012 Registration Fees:
$90 Registration fee for returning U5-U18 players (includes $10 fundraiser discount card).
$60 Registration fee for returning U4 players (includes $10 fundraiser discount card).
$105 Registration fee for NEW U5-U18 players (includes $10 fundraiser discount card).
$75 Registration fee for NEW U4 players (includes $10 fundraiser discount card).
$5 fee for Online Registrations.
$10 Late fee for all registrations on Jan. 28 or After
There will be a $25.00 fee for any returned check and payment must be in cash. ALL NEW PLAYERS MUST BRING A COPY OF THEIR BIRTH CERTIFICATE. If registering online, please mail the birth certificate to the address below.
Players born between August 1, 2007 and July 31, 2008 are eligible for our U4 division this season - No exceptions
Special Requests:
Only the Under 5 and Under 6 players may request a specific team or coach. CISA does not allow older players to be placed on a specific team. This is done to ensure that our coaches are not recruiting to build a strong team. CISA provides recreational soccer that allows players to have fun and learn the game.
Parents may request their child to play with a sibling. Parents may also request their child to play up one age level. Players cannot play down an age group.
Team Formation:
Teams will be formed after registration ends and given to the coach at the coach's meetings. Your coach will then contact you for the first team meeting/practice.
Coaches:
CISA has all volunteer coaches. Please let the appropriate board member know if you can coach. This is especially important in the Under 5 and Under 6 age groups.

No one will be gauranteed a spot on their previous team after Jan 21th.

If you need assistance with a login username and password please contact our Registrar. If you wish to play up one year, please list that information in the comments section. If you wish to not play on your previous Spring team, please state this in the comments section and you will be placed in the draft for a new team.
Registration by Mail

Refund Policy
Players are typically assigned to teams, uniforms purchased, and fees paid to NTSA, weeks in advance of practice and game scheduling. Therefore, once a player is registered, refunds will be at the sole discretion of the Board. There are NO REFUNDS for practice schedule conflicts, disagreement with coach, a decision not to play, or any other conflicts. A refund will be granted in the following situations:
1. The child cannot be placed on a team due to lack of players or coaches prior to the start of the season.
2. The child cannot play for medical reasons; a doctor's note must accompany the refund request.
3. At the sole discretion of the Board.
Refund requests must be in writing and either mailed to CISA at current address or emailed to a Board member.
New players must bring a copy of their birth certificate.
